Please use the form below to enter your information, noting that there are five pieces of information that we would like to have for each suggestion. If for some reason, you do not know all of the relevant information, then simply leave blank what you don't know, and we'll try to fill in the rest from our sources.

The first two pieces of information are the Name of the Book and the Name of the Book's Author or Primary Editor. The third and fourth pieces of information are the Name of the Book's Publisher and the Year of Publication. The final piece of information is the The Book's ISBN, or International Standard Book Number. Generally, we can identify a given publication by only two pieces of information, as follows: we will definitely need item no. 1 and after that, any one of items no. 2, 3, or 5 (yes.. technically the ISBN alone will uniquely identify the book, but we would appreciate a book title anyhow). Item no. 4 is helpful but not necessary.

Also please indicate the "department" within the bookstore into which this particular book should be filed. If you do not feel that any of the existing departments properly represent this book's content, then you may certainly recommend a new department as well.

Please note that the final acceptance of any and all book inclusions, and the creation of any new departments, are the sole discretion of WBH Associates, Inc.
